Several other studies have also reported a similarly high prevalence of low total and free testosterone levels in older men. In contrast, more recent studies, using liquid chromatography tandem mass found the prevalence of androgen deficiency to be significantly lower than that observed in the MMAS and BLSA (28,29,36-39). Although 10–15% of men aged ≥65 years have low total testosterone levels (Table 1) (36-39), the prevalence of late-onset hypogonadism defined by symptoms and a total testosterone level <8 nmol/l in the EMAS was 3.2% for men aged 60–69 years and 5.1% for those aged 70–79 years (36). The Healthy Man Study in Australia found no significant age-related decline in testosterone or dihydrotestosterone in men who reported being in good health (49). The authors of the Health Man Study have argued that ill health, rather than aging itself, is the major contributor to androgen deficiency in older men.
